Dear fifth grade families,

You will soon be getting an email from prompting you to complete and sign athletic and activities forms for your student(s).

�������ϲ�ƽ̨��վͶע partners with to manage our middle and high school athletics and activities registration.  Final Forms registration requires both parent/guardian e-signature and student e-signature.

As you plan for middle school, we want you to know about the fun activities students can choose to participate in.

Sixth graders can participate in the following sports and activities at no charge to families:

• Coed Track and Field – Fall
• Boys and Girls Soccer – Fall
• Coed Unified Bowling- Fall
• Coed Dance and Tumbling – Pre-Winter
• Coed Unified Basketball – Pre-Winter
• Coed Wrestling -Winter
• Coed Cross Country – Spring
• Girls Basketball – Spring
• Coed Unified Soccer- Spring
• Clubs: Each middle school has a long list of clubs and activities, which vary by school. Please contact your middle school’s office staff, listed below, for current offerings.

Participation in athletics requires students to have a current sports physical on file.

We encourage anyone interested in playing sports next year to get a sport’s physical early. If your family needs assistance getting a physical for your child, please visit our resources webpage or contact Family Support Services at 360-676-2790.

How to participate:

For first time users, when you receive the email from , click on the personalized link to get started. You can access . If you require any support during the process, scroll to the page bottom and click “Use Support.”

For returning users to be cleared for any of our 2024-2025 school year activities, go to to update your student’s information and documents.

1. Go to , click on “Parent” and log in.
   1. If you forgot your password, click “Forgot your password?”
   2. If you have never set up your  account, click “Never logged in?” then enter the main email address that we have on file for you in Skyward to resend the confirmation link. Then, use the link to set up the account and login.
2. Click on the blue box at the top of the page to register for 2024-25 school year. It looks like this:
3. Then, fill out the information or forms for each student you have in the system. Some things you may need to do are:
   1. Select sports your student is interested in participating in next year.
   2. Update forms and parent permissions. Most forms are fillable in Final Forms; you will need to upload a copy (photo, scan or PDF) of current sports physical if your student’s physical is not up to date. Once uploaded, physicals are good for two years.
   3. Sign each form with parent e-signature.
   4. Have your student login to with their school district email to update student e-signature that acknowledges compliance with �������ϲ�ƽ̨��վͶע and state policies for athletes.
